Neighborhood Connections Toolkit


Neighborhood Connections began as a Grant funded program at the Azusa City Library in 2015. The central feature of this program is to provide one-on-one information referral to patrons in need of specialized assistance. Since its debut, the program has assisted more than 900 people in the areas of housing, employment, veterans benefits, legal aid, citizenship services and more. In 2017 the Azusa City was awarded an LSTA" Pitch an Idea" grant to create a toolkit for this program so that other libraries can benefit from our experience. The toolkit includes templates, forms, sample job descriptions, and promotional brochures. A YouTube channel (Azusa Library) has also been established to showcase presentations.
