Apply for a Library Card
Age 18 & Older
Azusa City Library Cards are free to all California residents. Please bring the following to the library to get your free card:
- A completed Application Form
- Valid government ID issued in California
- Proof of current mailing address posted within the last 60 days (P.O. Box not acceptable)
- Telephone contact number
Under 18 Years of Age
A parent or guardian must be present to show identity and sign for a child under the age of 18 who is applying for a library card.
My First Library Card
Students who attend Azusa schools in grades K - 6 are eligible for a My First Library Card. Cardholders may check out up to 4 books. To apply for a My First Library card, an application and a parent/guardian ID is required.

Application for My First Library Card
Temporary Card
Adults 18 years of age and older can apply for a free temporary library card good for 6 months with a valid photo ID. This card enables users to check out up to 3 books along with free access to library computers.
Virtual Card
For patron interested in our electronic databases and Overdrive eBooks, we now have a virtual card good for 6 months. Please note, virtual cards cannot be used to check out physical materials from the Library such as books, DVDs, or other audiovisual materials.

Please bring the required documents from the section above. The fee to replace a lost library card is $3.
